Novel ways to preserve the peritoneal membrane.

چکیده
The underlying pathophysiology that leads to morphologic and functional changes in the peritoneal membrane over time among patients on peritoneal dialysis is not well understood. Many studies have been conducted to try to abrogate those changes and so preserve peritoneal function. Conventional interventions that attempt to accomplish that end include prevention of peritonitis, timely removal of the peritoneal catheter in the face of non-resolving peritonitis, use of biocompatible dialysate, and limitation of total glucose exposure by avoiding hypertonic dextrose solutions. Inhibition of the renin-angiotensin-aldosterone and vascular endothelial growth factor systems, peritoneal resting, combined peritoneal dialysis and hemodialysis, and N-acetylcysteine and gene therapy are more novel and experimental attempts to preserve the peritoneal membrane. We review the novel studies that have aimed to promote the health and function of this dialyzing membrane.

Developing a Device for Automated Peritoneal Dialysis
Background: Peritoneal dialysis (PD) is an efficient alternative for hemodialysis. Removal of waste metabolites and excess water from the blood circulation are the two main tasks done during dialysis. These are done by using a filter; in PD, the naturally semi-permeable peritoneal membrane acts as the filter. Automated PD machines control intelligently the operations to enhance the efficiency a...
متن کاملConventional versus biocompatible peritoneal dialysis fluids: more questions than answers?
The most important challenge in peritoneal dialysis (PD) is long-term preservation of peritoneal membrane structure and function. Introduction of dialysis fluids into the peritoneal cavity induces changes. These changes are related to duration of dialysis, occurrence of peritonitis and components of the dialysis solution. Bioincompatibility is considered to be the major cause of the development...
متن کاملWhat happens to the peritoneal membrane in long-term peritoneal dialysis?
Correspondence to: A. De Vriese, Renal Unit, University Hospital, OK12, De Pintelaan 185, Gent B-9000 Belgium. [email protected] S long-term peritoneal dialysis (PD) requires preservation of the transport function of the peritoneal membrane. Adequate dialysis is not only a matter of achieving the targets for Kt/Vurea and creatinine clearance, but also—and maybe even more importantly—of main...
متن کاملDissolved molecular hydrogen (H2) in Peritoneal Dialysis (PD) solutions preserves mesothelial cells and peritoneal membrane integrity
BACKGROUND Peritoneal dialysis (PD) is used as renal replacement therapy in patients with end-stage kidney disease. However, peritoneal membrane failure remains problematic and constitutes a critical cause of PD discontinuation. Recent studies have revealed the unique biological action of molecular hydrogen (H2) as an anti-oxidant, which ameliorates tissue injury. In the present study, we aimed...
متن کاملCytotoxic Glucose Degradation Products in Fluids for Peritoneal Dialysis
During the standard heat sterilization process of the lactate–buffered peritoneal dialysis solutions, glucose (an osmotic active substance) degrades to form compounds called glucose degradation products which are cytotoxic and affect the survival of the peritoneal membrane. This case presentation is based on an observation of 224 aseptic peritonitis cases of unknown etiology. For the purpose of...
